GROUP LIFE AND AD&D INSURANCE
ACLI provides basic life insurance and accident coverage, through MetLife, at no cost to you. The monthly premiums for your coverage are paid by ACLI, however, you must designate a beneficiary (PDF) for this benefit. ACLI provided life and accidental death and dismemberment insurance benefits are as follows:

OPTIONAL LIFE AND AD&D INSURANCE
Optional Life Insurance is an optional benefit offered as a financial resource to protect your family. Optional Life and AD&D Insurance (PDF) is provided through MetLife. Eligible employees may purchase one to five times annual salary up to a maximum benefit of $500,000. Optional AD&D coverage is also available up to the optional life amount. You may also purchase up to $50,000 in optional coverage for your spouse ($10k increments up to 50% of employee amount) and $2,000 in optional coverage for each child. Optional AD&D coverage for your spouse and child(ren) is available up to their optional life amount(s).
Premiums are based on age and the amount of coverage elected. If you choose to elect new coverage or, if you are currently enrolled in this program and wish to increase your coverage an Enrollment Application (PDF) must be completed. All optional life insurance elections made outside of the new hire window are subject to medical underwriting. The Statement of Health Form (PDF) must be completed and returned along with your Enrollment Application.
