WHAT TO DO WHEN STARTING AT ACLI
- Meet with Kellie Croons to sign up for your benefits.
- Set up your voice mail.
- Get to know the staff.
- Review ACLI’s Policies and Procedures.
- Review Finance’s Policies & Procedures.
- Need to learn more about insurance? Consider the LOMA program.
WHAT TO DO WHEN GETTING MARRIED
- Send email to Kellie Croons to notify HR of the change.
- Change Beneficiary Forms for Life Insurance and 401(k).
- If your name has changed, you should notify Social Security. Give copy of new social security card to HR.
- Update Emergency Contact Form and send completed form to Kellie Croons.
- Change tax forms and send completed forms to Andrew Baboyian.
Other things to think about:
- You can add or remove your spouse from your health and dental insurance.*
- You can sign up for or update your flexible spending account deductions.*
- You can sign up for or update your Dependent Life Coverage.*
*You must make these changes within 30 days from the date of the change in status.
WHAT TO DO WHEN YOU MOVE
- Send email to Kellie Croons to notify her of the new address, phone number and the effective date.
- Kellie will notify Aetna, Kaiser, Metlife, Prudential and TASC, as applicable.
- Update tax forms (if you move from one state/DC to another) and send completed form to Andrew Baboyian.
- If you participate in the FSA, you can also update your address on-line at Discovery Benefits.
WHAT TO DO WHEN HAVING A BABY
- Prior to having the baby, set up meeting with Kellie Croons to discuss disability and FMLA leave.
- Send email to Kellie Croons to add child to Health and Dental (must be done within 30 days of child’s birth date).
- Update beneficiary forms and send completed forms to Kellie Croons.
- Update tax forms and send completed forms to Andrew Baboyian.
WHAT TO DO WHEN ADOPTING A CHILD
- Prior to adoption, set up meeting with Kellie Croons to discuss adoption assistance.
- Once adoption is official, send email to Kellie Croons and follow the same steps listed under “Have a Baby”.
